For Certified Retail Membership: For Profit Businesses: Please provide a Confirmation of Aboriginality (COA) for the majority of business owners and/or a Membership Certificate of Supply Nation or your state Chamber of Commerce. We cannot process your application for business certification unless you provide EITHER a COA for the majority of owners OR Supply Nation or Chamber membership certificate. Not-for-profit businesses: ORIC registered organisations – ORIC businesses running a retail business do not require certificates (unless board members have changed). ACNC Organisations – will require Supply Nation status or COA for the majority of board members. For Retailer Member – Individual – Please provide as many documents as available. For Service Member Directory listing – this section is optional. Note: If the field has a * the field must have a file uploaded. Please upload a blank file to ensure the form will submit if a document is not considered mandatory above.

Membership certificate
